Have two or more ICD-10 visit diagnoses (one of which must be ambulatory) for Alzheimer's Disease or Alzheimer's Disease Related Dementias (AD/ADRD)

Summary
This trial aims to help people living with dementia and their care partners access community support and services to improve care, satisfaction and reduce hospital visits.
Who is the study for?
This trial is for people aged 66 and older diagnosed with Alzheimer's or related dementias, who have had multiple health visits for these conditions. Their care partners must be adults over 18. It's not open to patients under the age of 66.
What is being tested?
The study tests three interventions: Nurse-led Telephonic Care (NLTC), Emergency Care Redesign (ECR), and Community Paramedic-led Transitions Intervention (CPTI) to improve post-emergency care for dementia patients.
What are the potential side effects?
Since this trial involves non-medical interventions like coaching and community support, traditional side effects are not expected; however, participants may experience changes in their usual care routines.
